Suggestions coming from the August 24, 2010 critique regarding the 2010-08-04 Quad incident and the 2010-08-18 Table Mountain incident.

Pincher Creek SAR routinely performs a critique after a Search and Rescue incident in order to learn what should be done differently or the same in the next similar incident. Critiques are not about criticism but rather about learning. They are normally chaired by a Search Manager that was not involved in the incident. The suggestions that are brought forward through the critique process are presented in this report.

Suggestions

Critiques/2010-08-24/Sug/0008:

Suggestion #8: discuss (formally) with PCES the issues of navigation to call-out sites (for example: on the August 04 incident south of PC, the ambulance missed the directions.

Committee handling: preplan
Value to PCSAR mandate: medium
Difficulty to implement: easy

Action: Pre-plan to hold joint critique among SAR, Ambulance Services, EMS and Fire Dept..

How to say "local maps are wrong. This is how you get there." Local knowledge. Dispatch passing incomplete info.

Suggestion #20: EMS personnel were not outfitted to climb the mountain. They wore rubber boots, full cover-alls on a hot day.

Committee handling: unassigned
Value to PCSAR mandate: not rated
Difficulty to implement: done

EMS was available first. Came from Beaver Mines.

Critiques/2010-08-24/Sug/0021:

Suggestion #21: No PCSAR team leader in the field.

Committee handling: training
Value to PCSAR mandate: high
Difficulty to implement: easy

Original issue was "A bear had been seen before, but not all knew that a bear was in area." but the root cause was that a safety briefing was missed, in fact there was no briefing, because there was no team leader in the field.

Happens a lot when we are working with other agencies. Lack of our leadership when we work with PCES. We assume they will do it.

First member that arrives should assume leadership for our organization.

How to coordinate between 2 different agencies.

Who know that the bear was there? SRD knew? There's always a bear in that area. Was it a risk?

Action:

  • Train senior PCES in SAR approach.
  • General training - every team needs a leader.
    • Renew training for all members, including senior members, that whenever together must identify a team leader.

Preplan 2012-09-18

  • refer to train committee
  • lesson: every time you approach a group and its not obvious who is in charge, ask explicitly "who's in charge?"
    • if you get an ambiguous answer, ask for precision
    • encourage our partners to be explicit, clear cut
    • teams merging
      • team leaders shall have a quick discussion on which leader will be over-all for both teams
      • usually the leader of the first team on site

Critiques/2010-08-24/Sug/0022:

Suggestion #22: PCSAR & EMS joint training (witness a mock search). Would provide opportunity to let EMS personnel see capability of SAR.

Committee handling: training
Value to PCSAR mandate: medium
Difficulty to implement: medium

also preplan committee

Action:

  • Verify that they're interested
  • run session on one of their training days

Suggestion #23: working with EMS, need to get SAR into action faster

Committee handling: preplan
Value to PCSAR mandate: high
Difficulty to implement: medium
  • want to get response time lower
    • should marshal at fire hall
    • consider pagers
    • consider radios
    • consider one-way radios
    • consider call-out through dispatch

Sketch out a typical timeline for a rescue response. Focus on each element.

Get phone call out quicker.

Shell has a call-out system. ACRO uses a phone system that can play a message to everyones phone numbers in just a couple of minutes.

Could call back number have a PCSAR-owned voice mail box that any call-back person could review. Can receive and answer multiple calls at a time. Recorded "greeting" message gives updated/additional details. Members leave message only if they are able to respond.

Sign-in via call-out system.

Muster at site not firehall.

Suggestion #24: Consider using back door number to talk to Lethbridge dispatch to avoid long calls of questions

Committee handling: preplan
Value to PCSAR mandate: medium
Difficulty to implement: easy

When calling 911, there are sometimes delays when forced to answer questions that are not relevant, like background info on caller.

  • Dave will discuss with Chris
